MISSOURI (KBSI) – The Missouri Department of Social Services is seeking proposals from organizations to expand nurse-based home visiting services in rural communities through the state’s Rural Health Transformation Program.
The Missouri Children’s Trust Fund has released a request for proposals for organizations that currently provide, or are working to implement, evidence-based nurse home visiting programs.
The program will provide $588,000 annually to support the expansion of Home Visiting Evidence of Effectiveness-approved programs. Funding includes up to $100,000 for model fees and training for new nurse home visitors and up to $488,000 to expand existing services.
State officials said the investment is intended to improve maternal and child health, strengthen families and help prevent child abuse and neglect by increasing access to high-quality home visiting services in rural Missouri.
To qualify, applicants must:
- Operate or be actively implementing an evidence-based HomVEE-approved home visiting model.
- Demonstrate fidelity to the approved model.
- Provide services delivered by registered nurses.
- Use the funding to expand existing services rather than replace current funding.
Organizations interested in applying are encouraged to attend a virtual informational meeting from 9 to 10 a.m. July 8.
Applications are due by Aug. 23.
The Rural Health Transformation Program is funded through the U.S. Department of Health and Human Services’ Centers for Medicare & Medicaid Services. According to the Missouri Department of Social Services, the program is supported by a federal financial assistance award totaling more than $216.2 million.
